PHARMACIST-MANAGED WARFARIN CLINIC AT HOSPITAL DUCHESS OF KENT – HAVE WE IMPROVED OVER THE YEARS?

ABSTRACT

Introduction: The HDOK pharmacist-managed warfarin clinic (PMWC) was started in December 2005. Lau et al (2006) found that anticoagulation control by the PMWC was as good as usual medical care. There were significant improvement in compliance and patient knowledge. Although monthly audits are carried out, in-depth study on anticoagulation control had not been undertaken since 2006.


Objective: To evaluate if outpatient INR control in the PMWC have improved with experience and changes in protocol/workflow over the years


Methodology: Retrospective medical record review, 4 months in duration, investigating INR control in 2006 and 2009.


Results: 131 and 191 patients were studied in the 2006 and 2009 periods respectively. (to be cont)


Conclusion: After three years of experience running the warfarin clinic, the PMWC significantly improved anticoagulation control and reduced the number of INR tests required to maintain good INR control.
