Just a glimpse of my work.
S- Subject. Ok or not. Comfortable or not. Any complains.
O- Objectives. These are definite information, INR, lab data and also BP. (yes, we pharmacist measure BP using the sphygmomanometer)
A- Assestment. Whether INR within range, any problems encountered, any bleeding, any missed dose, etc.
P- Plan. Increase, maintain or decrease dose, TCA, etc....
This is one of the shorter SOAPs, so I put this up to protect patient confidentiality.
Notice "Hospital Duchess of Kent"?? Haha.
